Use proper ReST syntax for chapters. Add more information to enhance
standardization in the file and to make the rendering more homogeneous.
Add a SPDX identifier. Mark single-queue mode as deprecated.

Rename the block documentation files to ReST, add an
index for them and adjust in order to produce a nice html
output via the Sphinx build system.

At its new index.rst, let's add a :orphan: while this is not linked to
the main index.rst file, in order to avoid build warnings.

The ELV_MQUEUE_XXX definitions in include/linux/elevator.h are unused
since the removal of elevator_may_queue_fn in kernel 5.0. Remove these
definitions and also remove the documentation of elevator_may_queue_fn
in Documentiation/block/biodoc.txt.

   - block opal shadow mbr additions (Jonas, Revanth)

   - sbitmap compare-and-exhange improvemnts (Pavel)

   - Fix for potential bio-&gt;bi_size overflow (Ming)

   - NVMe pull requests:
       - improved PCIe suspent support (Keith Busch)
       - error injection support for the admin queue (Akinobu Mita)
       - Fibre Channel discovery improvements (James Smart)
       - tracing improvements including nvmetc tracing support (Minwoo Im)
