This patch adds new capture board Hawell HW-9004V1. This card has 4 SAA71300 chips. In order to work it is needed to initialize its registers (gpio mask and value). Documentation: remove __dev* attributes.

CONFIG_HOTPLUG is going away as an option. As a result, the __dev* markings need to be removed. This change removes the use of __devinit, __devexit_p, __devinitdata, __devinitconst, and __devexit from the kernel documentation.
