<title>MIPS: Apply `.insn' to fixup labels throughout</title>

Fix the issue with the ISA bit being lost in fixups that jump to labels
placed just before a section switch.  Such a switch leads to the ISA bit
being lost, because GAS concludes there is no code that follows and
therefore the label refers to data.  Use the `.insn' pseudo-op to
convince the tool this is not the case.

This lack of label annotation leads to microMIPS compilation errors
like:

mips-linux-gnu-ld: arch/mips/built-in.o: .fixup+0x3b8: Unsupported jump between ISA modes; consider recompiling with interlinking enabled.
mips-linux-gnu-ld: final link failed: Bad value

<title>MIPS: Fix gigaton of warning building with microMIPS.</title>

With binutils 2.24 the attempt to switch with microMIPS mode to MIPS III
mode through .set mips3 results in *lots* of warnings like

{standard input}: Assembler messages:
{standard input}:397: Warning: the 64-bit MIPS architecture does not support the `smartmips' extension

during a kernel build.  Fixed by using .set arch=r4000 instead.

This breaks support for building the kernel with binutils 2.13 which
was supported for 32 bit kernels only anyway and 2.14 which was a bad
vintage for MIPS anyway.

Pull MIPS updates from Ralf Baechle:

 o Add basic support for the Mediatek/Ralink Wireless SoC family.

 o The Qualcomm Atheros platform is extended by support for the new
   QCA955X SoC series as well as a bunch of patches that get the code
   ready for OF support.

 o Lantiq and BCM47XX platform have a few improvements and bug fixes.

 o MIPS has sent a few patches that get the kernel ready for the
   upcoming microMIPS support.

 o The rest of the series is made up of small bug fixes and cleanups
   that relate to various parts of the MIPS code.  The biggy in there is
   a whitespace cleanup.  After I was sent another set of whitespace
   cleanup patches I decided it was the time to clean the whitespace
   "issues" for once and and that touches many files below arch/mips/.

Fix up silly conflicts, mostly due to whitespace cleanups.
