Pull SCSI updates from James Bottomley:
 "This series consists of the usual driver updates (ufs, qla2xxx,
  target, smartpqi, lpfc, mpt3sas).

  The core change causing the most churn was replacing the command
  request field request with a macro, allowing us to offset map to it
  and remove the redundant field; the same was also done for the tag
  field.

  The most impactful change is the final removal of scsi_ioctl, which
  has been deprecated for over a decade"

blk_mq_alloc_disk() returns error pointers, it doesn't return NULL
so correct the check.

Only the sr driver can handle SCSI passthrough requests, so move the call
to scsi_cmd_blk_ioctl() there.

-&gt;revalidate_disk is only called during add_disk for pd, but at that
point the driver has already set the capacity to the one returned from
Identify a little earlier, so this additional update is entirely
superflous.

We can remove 'q' from blk_execute_rq as well after the previous change
in blk_execute_rq_nowait.

And more importantly it never really was needed to start with given
that we can trivial derive it from struct request.

The pcd driver does not have a -&gt;revalidate_disk method, so it can just
use bdev_check_media_change without any additional changes.
