drivers/lightnvm: fix a null-ptr-deref bug in pblk-core.c

At line 294 in drivers/lightnvm/pblk-write.c, function pblk_gen_run_ws is called with actual param GFP_ATOMIC. pblk_gen_run_ws call mempool_alloc using "GFP_ATOMIC" flag, so mempool_alloc can return null. lightnvm: fix out-of-bounds write to array devices->info[]

There is an off-by-one array check that can lead to a out-of-bounds write to devices->info[i]. Fix this by checking by using >= rather than > for the size check. Also replace hard-coded array size limit with ARRAY_SIZE on the array. The make_request_fn is a little weird in that it sits directly in struct request_queue instead of an operation vector. Replace it with a block_device_operations method called submit_bio (which describes much better what it does).
