hvc_xen: fix xenboot for DomUs

The xenboot early console has been partially broken for DomU for a long time: the output would only go to the hypervisor via hypercall (HYPERVISOR_console_io), while it wouldn't actually go to the DomU console. The reason is that domU_write_console would return early as no xencons structs are configured for it. Add an appropriate xencons struct for xenboot from the xenboot setup callback. hvc_xen: add earlycon support

Introduce EARLYCON support in hvc_xen, useful for early debugging on arm and arm64, where xen early_printk is not available. xencons_disconnect_backend() is only called from xen_console_remove(), which is conditionally compiled, so we get a harmless warning when CONFIG_HVC_XEN_FRONTEND is unset: hvc/hvc_xen.c:350:12: error: 'xen_console_remove' defined but not used [-Werror=unused-function] This moves the function down into the same #ifdef section to silence the warning. drivers/tty: make hvc/hvc_vio.c explicitly non-modular

The Kconfig currently controlling compilation of this code is: drivers/tty/hvc/Kconfig:config HVC_CONSOLE drivers/tty/hvc/Kconfig: bool "pSeries Hypervisor Virtual Console support" ...meaning that it currently is not being built as a module by anyone. Lets remove the modular code that is essentially orphaned, so that when reading the driver there is no doubt it is builtin-only. Since module_init translates to device_initcall in the non-modular case, the init ordering remains unchanged with this commit. We explicitly disallow a driver unbind, since that doesn't have a sensible use case anyway, and it allows us to drop the ".remove" code for non-modular drivers. We don't replace module.h with init.h since the file already has that. Also note that MODULE_DEVICE_TABLE is a no-op for non-modular code.
