commit 93825f2ec736 converted NSEC_PER_SEC to TICK_NSEC because the author
confused NSEC_PER_JIFFY with NSEC_PER_SEC.

As a result, the calculation of refined jiffies got broken, triggering
lockups.

Fixes: 93825f2ec736 ("jiffies: Reuse TICK_NSEC instead of NSEC_PER_JIFFY")

Introduce a trivial, mostly empty <linux/sched/cputime.h> header
to prepare for the moving of cputime functionality out of sched.h.

Update all code that relies on these facilities.

We are going to split <linux/sched/debug.h> out of <linux/sched.h>, which
will have to be picked up from other headers and a couple of .c files.

Create a trivial placeholder <linux/sched/debug.h> file that just
maps to <linux/sched.h> to make this patch obviously correct and
bisectable.

Include the new header in the files that are going to need it.
