qed: Use enum as per FW 8.59.1.0 in qed_iro_hsi.h
qed_iro_hsi.h contains HSI changes related to storm memories access.
Existing code is based on hard-coded index.
Use enum as defined for FW HSI 8.59.1.0, instead of hard-coded index.
This patch also removes unnecessary header file inclusion.
