intel_th: pci: Use PCI_DEVICE_DATA() for device entries
Switch PCI_DEVICE() macro to PCI_DEVICE_DATA() to reduce the number of
lines. Add a new header file for the defined names of the devices. Sort
the entries in ascending order.
