KVM: x86: Bury ioapic.h definitions behind CONFIG_KVM_IOAPIC
Now that almost everything in ioapic.h is used only by code guarded by
CONFIG_KVM_IOAPIC=y, bury (almost) the entire thing behind the Kconfig.
